Frequently Asked Questions about Registration Note: Members from new communities will receive access to the site and instructions on how to use at the Coaches Meeting held in early March. Prior to that meeting, you are welcome to use the public portion of the site but will not have access to private materials reached through the registration/log-in process. a. Go to www.achievecommunities.org. Select Log In located along the top toolbar. Select Register as a New User. 2. Who is eligible to register? a. Any Coach, Community Health Action Response Team (CHART) member, or affiliated national partner organization member can register to use the ACHIEVE site. 3. I have not previously registered as a member of the ACHIEVE site. But when I enter my email address, it is asking me for a password. What is happening? a. The ACHIEVE website is hosted by the National Association for County and City Health Officials (NACCHO). It is possible that while you have not registered for the ACHIEVE site, you may have previously registered for NACCHO’s wen>bsite and are being found in the system. If you remember your NACCHO password, please enter it to access the site. You will then be directed to respond to specific ACHIEVE-based questions to determine your level of access. If you do not remember your NACCHO password, please select Forgot Password. You should receive an email providing you with further directions for changing your password. If you are sure you do not have a NACCHO account or do not receive an email after selecting Forgot Password, please email info@achievecommunities.org with your issue. Include a description of the problem, your national partner organization, and your affiliated community name. 4. I registered for the ACHIEVE site but never received notification that I received access. What should I do? a. First, check to see if you have access to the site by returning to www.achievecommunities.org. Select Log In. Select Returning User. Enter your log in and password information. If you are redirected to the collaboration homepage, then you have received access to the site. If you are not redirected to the page, you have not yet been approved and may still be in the approval process. If you registered more than five business days ago and still have not received access, please email info@achievecommunities.org with your issue. Include a description of the problem, your national partner organization, and your affiliated community name. 5. I registered for the ACHIEVE site, received an email saying I now had access, however when I return to the site to log in, it is still asking me for my contact information. What should I do? a. Returning users are asked to verify their contact information. When you attempt to log in to the site, you should expect to see your pre-filled contact information appear. If nothing has changed, simply click through the registration screens. If your information has changed, please make those changes on the form before proceeding. 6. I registered for the ACHIEVE site and received an email saying I now have access. How do I log in? a. Return to www.achievecommunities.org. Select Log In from the top toolbar. Select Returning User. Enter your login and password. You should now be directed to the collaboration homepage. 7. My contact information has changed since registration. How do I make these changes? a. Every time you enter the site, you will be prompted to verify your contact information and affiliated community. Change your information on the form when it appears and then proceed to access the site. 8. I have forgotten my username and /or password. What should I do? a. Your username is the email address you registered for the site with. Select Forgot Password for additional information about how to change your password to be emailed to you. 9. How do I move back and forth between the public and collaboration portions of the site? a. When you log in to the site, you will automatically be taken to the collaboration homepage. To return to the public portion of the site, click on the large ACHIEVE logo at the top left of the page. To return to the collaboration site, click on Log In again. 10. How do I know what partner organization and community to select during the registration process? a. Your partner organization is the organization you are receiving funding from. Your community is the community you are either a Coach or CHART team member of. If you are a partner, do not select a community. Everyone should select a partner organization regardless of what their ACHIEVE role is. 11. I am a state health department expert advisor- what do I select? a. If you are a state health department expert advisor, please select both the national organization (NACDD) and community you are affiliated with. Your registration will be the same as a Coach. If you are working with more than one community in your state, please contact David Yum at NACDD. 12. I selected the wrong national partner organization or community during the registration process. How do I fix this? a. Go back to the site and re-register with the correct information. 13. I am from the Winter Park, FL community. How do I register? a. Your registration will be the same as any other community. |
