Community Health Resources Database - Developed by CDC's Community Health and Program Services Branch, the database includes links to hundreds of useful planning guides, evaluation frameworks, communication materials, health risk factors data and statistics, fact sheets, scientific articles, and key reports from a variety of organizations, much of which is geared toward supporting policy changes in communities.

Recommended Community Strategies and Measurements to Prevent Obesity in the United States - This CDC report contains 24 recommended obesity prevention strategies focusing on environmental- and policy-level changes that can be implemented by local governments and school districts to promote healthy eating and active living.

Tools for Community Action from CDC's Healthy Communities Program – Resources include the Community Health Assessment and Group Evaluation (CHANGE) Tool and several Action Guides that provide communities with “how-to” guidance for implementing effective community-level health promotion strategies.

Community Health Assessment aNd Group Evaluation (CHANGE): Building a Foundation of Knowledge to Prioritize Community Health Needs- CDC’s Healthy Communities Program identified a need for new ways of assessing and documenting policy, systems, and environmental change in communities. After reviewing community assessment literature and local and state assessment tools and receiving internal and external partner input, the Healthy Communities Program developed Community Health Assessment aNd Group Evaluation (CHANGE). The tool was then pilot tested with funded communities, who provided valuable feedback in making the final product a more user-friendly tool.
